Learning Disabilities

Season Healthcare Living provides personalised services which are reflected in our support packages - which will be (always) individually tailored to meet the specific needs of each service user.

The overall aim is supporting our service users to reach their full potential and this is achieved by facilitating the learning of new skills and confidence building, whilst embracing the value of learning.

Autism & Differing Behaviours

This will be delivered using the PROACT-SCIPr-UK Model. PROACT SCIPr-UK® is a holistic approach used when working with differing behaviours. Support workers are trained to identify triggers and recognise early behavioural indicators, so that appropriate interventions can be used to prevent a crisis from occurring. The proactive style improves the quality of life for our customers and their families; reducing social barriers and enabling a more active & fulfilling life.

Mental Health & Brain Injury

Season Healthcare support workers are specifically trained to meet the needs of our service users with a range of mental health needs, varying from mild anxiety to severe mental health problems.
At Season Healthcare we understand that life after a brain injury or the diagnosis of a long-term neurological condition can feel challenging and confusing. Season Healthcare works with service users together with their families and friends to support and enable them to reach their full potential.

Reablement

Our Specialist Reablement Services provides recuperative support to people following a hospital admission and during rehabilitaion. This multidisciplinary intermediate care service was designed due to recognising the crucial role to play in supporting people to recover and gain independence including preventing the need for re-addmission to hospital

In an effort to obtain as much information as possible about the views and experiences of service users and their representatives, we undertake the following:

Service users will receive three monthly visits from their dedicated care manager to ensure that the services being provided are meeting service user needs.

Supporting adults with learning disabilities with a natural person-centred transition towards greater independence.

The Season Healthcare outreach service provides quality care and support possible at home and in the community for people with mild, moderate and severe learning disabilities. We believe in providing a personalised service that is reflected in our support packages.

In an effort to obtain as much information as possible about the views and experiences of service users and their representatives, we undertake the following:

As a minimum, each service user will receive an annual comprehensive service review and reassessment of their needs carried out by their appointed care manager. In the event that a service needs change in between these annual reviews, more frequent reassessments will gladly be undertaken.

Six monthly quality assurance surveys are sent to all service users or where more appropriate, to their representatives to obtain feedback on our service performance.

Annual surveys are undertaken with team members to understand how we are doing and what can be done to improve the lives of our service users and the team.

Operate a 24 hour, 7 day a week on-call service to ensure that a senior member of the team is always available to assist both service users and team members caring for clients.

Provide a comprehensive induction programme as well as ongoing training to all Season Healthcare team members. Participate fully in all inspections carried out by the Care Quality Commission, as required under our registration as a registered care provider.
