OUR APPROACH

  • We put the individual at the heart of what we do. We listen, learn and build relationships, providing support that’s unique to each person.
  • We apply a strength-based approach, where care workers undertake agreed tasks with individuals, as opposed to doing everything for them when possible. This is where skilled staff really can make a difference to someone’s life.
  • Rest assured that the carers assigned to individual care packages are fully trained to meet any specific needs.
  • Our teams work either Monday to Friday, days, nights, weekends, or evenings, and we pre-plan rotas so our service users and their families always know who is coming. However, in the rare event a carer has to cancel because of illness or an emergency, additional care workers who are on call can quickly cover.
  • Our staff are multi-culturally diverse. Staff are matched to service users according to relevant skills but of course, our service users have a high degree of agency in choosing someone they are comfortable with. This enables us to enhance communication, develop positive rapport and provide tailored, empowering support.

Season Healthcare's team of care managers WILL:

  • Organise the care packages needed to facilitate service users in fulfilling their goals and emotional and physical needs. We always put you at the centre of your care planning.
  • Work with families and friends of service users to build strong, supportive relationships.
  • Communicate and liaise with healthcare organisations regarding the care and support required and received.
  • Keep abreast of legislative changes in the sector.
  • Review your care package annually or before if needed.

Your care plan

Season Healthcare Living’s care plans are created to fully support individuals through strength-based, personalised support to meet each person’s diverse and assessed health needs.

Each care package has an assigned senior carer and a carefully selected small team of care workers with predominantly shift-based working to ensure continuity of care.

All risk assessments are undertaken collaboratively with individuals and healthcare partners.

Each care plan also looks at decision-making in line with the law and how to promote best interests when there is limited or fluctuating capacity.

The care plan operates as a living document, which can be amended as outcomes are reached, milestones are achieved, or individual needs change.

As a minimum, we undertake an internal review every three months of all care packages and then a six-monthly review with our service users or their families to ensure that all needs are being met.

Our services are available 24 hours a day, every day.
